Chris, I think the tape was the problem. Using the double stick tape didn't cover the leading edge of the cloth and thus the threads could be picked up in the rollers.

PRECAUTION 1: Make sure that the tape will not melt by the heat of the fuser. I test ran paper taped with the scotch tape first and it worked OK. no melting odors, etc.

PRECAUTION 2: BE SURE that you tape the leading edge of the cloth down securely, covering all of the loose ends. Also freshly and cleanlyu cut the cloth so it doesn't have any edge ravels. I cut the 1.5 oz. cloth with a rotary cutter and followed the grain carefully, then handled it gently until I ran it through the machine.

Also, Don't run this through anyone elses machine!!!

In the meantime, if you can get the fiberglass strands cleaned out of the machine, it will probably be OK. (except for if the adhesive from the double faced tape gummed things up. Then there is more cleaning :>).
