Boy, it sure sounds like classic fisheye to me. The reason all the craters are on the flat surfaces is that sagging closes up the craters on the sides. It looks like a crater with a raised, smooth rim around it, and can be caused by any oily contaminant, but most likely blushing of the epoxy underneath. The areas could be quite large, looking like valleys as well.
I had some doozies in my roller tray cover, but only small craters on my deck. I suspect you'll have to scrub & sand and apply another coat.
