I know someone who works with handicapped kids in skiing and other outdoor pursuits, who is also a paddler. I will find out if she has any advice.
In the meantime, ask your friend if she uses that hand for pulling, and, if so, how she goes about it. That way we could come up with something that complements the adaptations she has already come up with.
From the responses you've gotten, it looks like a design contest is shaping up!
Another thought: there is a loose organization of people who solve these sorts of problems, I think it is called "Tetris" (don't trust that spelling). They are on the web and may already have a solution. I'll look them up tomorrow when I have time.
