well. if a knife falls out of the sheat then the sheat isnt a good one. Some are a pouch kind (no clip buttoned strip at the back) and tehy hold the knife quite nicely, yet its ready for use, but tethering a knife its like adding a dangerous line that could be difficult to cut since it could prevent the knife to move freely to cut it, a little lanyard or loop its enough. Mine has a cork handle (scandinavian fishermen use that a lot apparently) so it floats, that reduces the chances of loosing it.
I used to carry it when i was used to paraglide, but honestly never thought about switching to an hook to keep on me, maybe i should really do that and
just keep the knife in the boat, less danger of getting into trouble for carrying a blade too...
