I carry a fairly large diver's knife in such a way that nobody would notice it. Namely, by sewing my equipment carrying bags (VHF, GPS) onto my PFD. For the GPS bag I only sewed the sides onto the PFD, not the bottom. This leaves an opening between the GPS bag and the PFD where the knife blade goes, pointing up. The sheath of the knife is sewn onto the PFD underneath the GPS bag. All you can see is the handle. This works best with cheap PFDs that don't have many pockets. Sewing is a very good way of having a tight custom for for the equipment that you want to carry on your PFD.

I never had anybody complain about my knife except when I pulled it out to show them and they noticed how big it was. I never use it for anything by the way, it's just sitting on my PFD in case of emergency. What scares me a bit is that it might come out by accident when I do a self-rescue or something and accidentally press the release. To prevent this I wrapped a small bungee cord around the handle so you really have to pull to get it out.
